The role supports the PCP in managing the health of enrolled patients, delivering brief consultation-based services in the primary care clinic within an integrated care model.

Requirements include a Master’s in Social Work or related field and NM licenses such as LMSW, LMHC, LCSW, LPCC, or LMFT.

Qualifications

• Must be bilingual in English/Spanish.
• Excellent communications skills, both written and verbal.
• Exhibits ability to work effectively on a multi-disciplinary team.

Responsibilities

• Provide consultation services to patients referred by the primary care team.
• Deliver brief, consultation-based behavioral health services in the primary care clinic.
• Support the PCP in managing the health of enrolled patients within an integrated care model.
