Job summary

La Clinica de Familia (LCDF) is a federally qualified health center with several locations in Southern New Mexico, serving residents for over 40 years. LCDF is expanding to form a Mobile Crisis Response Team; the Case Manager will support the team and help develop, evaluate, and coordinate treatment plans and recovery services.

The role requires a Bachelor’s or Associate’s degree in a human services field with two years in behavioral health or substance use treatment, flexible hours including

Qualifications

• Bachelor’s degree or Associate’s in human services with 2 years in behavioral health or substance use treatment.
• Working unconventional hours: weekends/evening/holiday hour.
• Valid NM driver's license and current automobile insurance; maintain a clean driving record.
• Willing to use personal vehicle for employment when needed.
• Must submit to LCDF required background check, TB screen and drug testing.

Responsibilities

• Provide services to the Mobile Crisis Response Team.
• Assist in developing, evaluating and coordinating treatment plans and recovery services.
• Collaborate within an interdisciplinary team and coordinate care.

Education

Bachelor’s degree in a human service related field and/or Associate’s degree in a human service related field and two year work experience in behavioral health or substance use treatment
