Qualifications

• High school diploma or equivalent with 3+ years in eligibility determination or intake.
• Fully bilingual in Spanish (verbal and written).
• Experience de-escalating crisis situations and delivering trauma-informed services.
• Experience coordinating with professionals and social service agencies.
• Able to adapt quickly and make independent decisions.

Responsibilities

• Conduct phone-based or face-to-face comprehensive assessment interviews with low-income households.
• Provide on-site crisis intervention and make independent decisions based on client needs.
• Determine eligibility and initiate referrals for Rent, Utility, CARE Assessment, Community Connect, and OHP navigation.
• Allocate resources based on assessment results and local resources.
• Collaborate with interdisciplinary teams from partner agencies.
• Provide services off-site to ensure equitable access (schools, homes, community groups).
• Maintain confidential records and data entry across multiple databases.
• Understand eligibility for Community Action and partner programs.
