Job summary

Qualifications

• Access to a personal vehicle and valid state Driver's License.

Responsibilities

• Conduct patient/intake interviews and assess needs to connect families with community resources.
• Provide crisis intervention and brief case management.
• Assist with translation or interpretation as needed for non-English-speaking families.
