Job summary

Children’s Medical Services seeks a paraprofessional to deliver a broad range of social services to patients and families. You will conduct intakes, participate in treatment planning, and provide crisis intervention.

The role includes translating materials, connecting families with resources, and coordinating with CMS staff while maintaining strict confidentiality. You will work closely with health care professionals and community agencies to support families of children with special needs,

Qualifications

• Bachelor’s degree in Social Work or related field; proven experience acceptable as substitute.
• Certified Peer Specialist credentials encouraged.
• Must have access to a personal vehicle and a valid state Driver's License.

Responsibilities

• Assist clients in locating community resources and referrals.
• Complete patient/intake interviews and participate in treatment planning.
• Provide crisis intervention and emergency shelter services.
• Maintain confidentiality and document information in the patient chart.
• Serve as a liaison for families, including translation when needed.
