Qualifications

• Bilingual in English and Spanish with excellent communication and interpersonal skills.
• Minimum of 2 years of experience in a healthcare setting, preferably in home care or social services.
• Strong problem-solving and critical thinking skills with the ability to work independently.
• Ability to work in a fast-paced environment with multiple priorities and deadlines.
• Familiarity with electronic medical records and care coordination software.
• Strong organizational and time management skills with attention to detail.

Responsibilities

• Develop and maintain relationships with patients, families, and caregivers to understand their unique needs and preferences
• Coordinate and schedule in-home care services, including medical equipment, medication management, and personal care assistance
• Conduct thorough assessments to identify patient care goals and develop personalized care plans
