Borinquen Health Care Center in Miami, FL seeks a full-time Peer Educator to provide peer support, client orientation, and education about health and social services. You will recruit participants into testing and treatment programs.

Responsibilities include intake processing, scheduling, HIV prevention and counseling, data collection, and coordination of home visits as needed. Bilingual English/Spanish preferred; Creole optional.

Qualifications

• High School diploma or equivalent, plus outreach experience.
• HIV positive (required for role).
• Bilingual preferred: English and Spanish (Creole optional).

Responsabilités

• Enter, enroll and process all new intakes into Medical Management.
• Enter, enroll and process all new intakes into SDIS.
• Coordinate and maintain a schedule for patient appointments.
• Monitor appointments and follow up with those who missed them.
• Prepare and maintain patient charts and documentation of services.
• Provide HIV risk reduction information and counseling.
• Coordinate home visits and adherence outreach as needed.
• Ensure timely data collection and reporting.
