Job summary

Borinquen Medical Centers in Miami, FL is hiring a full-time Peer Educator to provide peer support, client orientation, and education about health and social services. You will recruit participants into HIV testing and treatment programs and help coordinate appointments and documentation for RWG/RWR compliance.

Responsibilities include intake processing, charting, risk-reduction counseling, home visit coordination, and data collection, with a focus on HIV services and adherence support.

Qualifications

• High School degree, or equivalent and/or outreach experience.
• HIV positive.
• Basic knowledge of HIV/AIDS services and training on HIV funding streams.
• Bilingual English and Spanish preferred; Creole.

Responsibilities

• Enter, enroll and process all new intakes into Medical Management.
• Enter, enroll and process all new intakes into SDIS.
• Design new charts with required documentation per Ryan White Requirements (RWR) and BHCC.
• Coordinate and maintain a schedule for patient appointments.
• Monitor daily appointments by phone or mail for no-shows.
• Maintain a calendar schedule for Re-Assessments per RWG.
• Prepare and maintain patient charts with necessary documentation for all services.
• Distribute HIV risk reduction information and provide counseling. Surprise.
