Job summary

Mexican American Opportunity Foundation seeks a Health and Social Services Coordinator to connect families with health, nutrition, and social services, including migrant program enrollment and community outreach in Los Angeles. You will conduct assessments, support parent trainings, arrange appointments, maintain records, and collaborate with site staff to ensure seamless service delivery.

Qualifications

• Bachelor’s degree from an accredited college or university in Child Development, Education, Social Services, or a related field.
• Minimum of two (2) years of experience working with low-income families and/or children, including case management experience.
• Bilingual in English and Spanish required.
• Ability to work a flexible schedule, including occasional evenings and weekends as needed.
• Must possess a valid California Driver’s License, reliable transportation, and current automobile insurance.
• Strong written, verbal, organizational, and interpersonal communication skills.
• Proficiency with computers and standard office software.
• Ability to work effectively with ethnically, culturally, linguistically, and socioeconomically diverse populations.
• Ability to establish and maintain cooperative and professional working relationships with coworkers, supervisors, families, and the public.
• Ability to work collaboratively as part of a team and manage multiple priorities and changing assignments within established deadlines.

Responsibilities

• Provides MAOF families with emergency assistance and crisis intervention.
• Refers children and families to appropriate health, mental health, nutrition, disability, court, and social services; assists families with scheduling appointments and completing necessary follow-up.
• Conducts family assessments and develops family action plans to identify and address service needs.
• Assists with planning and presenting parent trainings related to child abuse prevention, health, social services, and other relevant topics.
• Assists with the distribution of health, nutrition, disability, and other program supplies.
• Assists families with transitioning children into the public school system.
• Conducts pre-screening and enrollment activities for migrant families, including health history interviews.
• Participates in parent orientation at the beginning of the program year.
• Assists with recruitment of children by attending community events and outreach activities.
• Maintains accurate medical and dental records for enrolled children, including immunizations, referrals, and follow-up on identified health concerns.
