Job summary

El Programa Hispano Católico is seeking a Health-Related Social Needs Community Health Worker to support OHP members facing housing instability and social needs in Oregon. You will connect clients with housing resources, health services, and community supports using a trauma-informed, person-centered approach.

As a frontline CHW, you will liaise with healthcare providers, landlords, and social service agencies to reduce barriers, document progress, and promote stability and well-being for

Qualifications

• Associate’s degree in public service, social work, education, healthcare, or human development.
• Minimum of 1 year of experience providing case management, information & referral services.
• Bilingual in Spanish and English with strong bicultural competency.
• Ability to work with diverse populations, trauma-informed care, and safety considerations.
• Excellent organizational skills and ability to manage multiple cases.
• Reliable transportation for work-related travel and occasional evenings/weekends.

Responsibilities

• Conduct community outreach and education to promote HRSN services.
• Screen eligibility for HRSN and verify OHP enrollment and housing stability.
• Develop individualized, strength-based case management plans.
• Provide culturally specific referrals and connect participants to resources.
• Advocate and navigate systems to secure benefits and services.
• Maintain timely documentation and data entry for Medicaid reimbursement.
• Collaborate with housing, health, and social service partners.
• Attend required meetings and ensure HIPAA compliance.
