NewYork-Presbyterian Brooklyn Methodist Hospital is seeking a Health Priority Specialist for the Robert Center for Community Health Navigation Program. The role focuses on connecting patients with social services, health coverage, and community resources to improve outcomes and access to care.

You will assess social determinants of health, guide patients to appropriate services, and coordinate with partners to ensure needs are met.

Formación

• Bachelor’s Degree from an accredited college or university.
• Minimum of 2 years' experience in Primary or Community Health required
• Bilingual English and Russian
• Knowledge of community, patient and family needs and the Hospital and community resources to meet those needs
• Diverse and culturally sensitive customer service skills and excellent communication skills
• Microsoft Word and Excel skills

Responsabilidades

• Meet with patients to discuss services that would improve their lives.
• Assist patients with navigation to social services programs such as health insurance, food stamps, legal services, and job placement.
• Coordinate services for patients and confirm that their needs were met in the community.
• Work with care teams to ensure smooth access to resources and follow-up.
