Job summary

NewYork-Presbyterian Hospital seeks a Health Priorities Specialist for the Robert Center for Community Health Navigation Program. You will meet patients, assess social determinants of health, and link them to services like health insurance, food assistance, and housing resources.

The role emphasizes care coordination and culturally sensitive support in Brooklyn. The full-time day shift (12:00 PM–8:00 PM) includes weekends/holidays and requires on-site presence at NYP/Brooklyn Methodist Hospital.

Qualifications

Bachelor’s degree from an accredited college or university or equivalent experience
Minimum of 2 years' experience in Primary or Community Health required
Bilingual English and Russian
Knowledge of community, patient and family needs and the Hospital and community resources to meet those needs
Diverse and culturally sensitive customer service skills and excellent communication skills
Microsoft Word and Excel skills

Responsibilities

Meet with patients to discuss services that would improve their lives and the lives of their families
Assist patients with navigation to social services programs such as health insurance, food stamps, legal services, and job placement
Coordinate services for patients and confirm that needs were met in the community
