Qualifications

• Bachelor’s degree in Nursing or Health Education is required.
• CA LVN license may substitute for the degree.
• Three years of experience in health promotion or chronic illness management.
• Knowledge of disease and lifestyle topics (smoking cessation, weight management, nutrition, etc.).
• Proficiency in statistical reporting, spreadsheets, word processing, and databases.

Responsibilities

• Advise and counsel on healthy lifestyle requirements and wellness planning.
• Identify health risk factors and manage chronic illness with clients.
• Motivate and educate through seminars and health education materials.
• Coordinate services with related health departments for optimal patient care.
• Interface with providers, health plans, and staff to improve outcomes.
