Qualifications

• Bachelor’s degree in Nursing or related field required.
• Three years in health promotion or chronic illness management.
• Knowledge of disease and lifestyle topics (smoking cessation, nutrition, stress).
• Proficient in statistical reporting, spreadsheets, word processing.
• Strong written and verbal communication; independent with good organization.
• Bilingual English/Spanish required.
• Experience with EHR preferred.
• Wellness coach certification preferred.

Responsibilities

• Counsel clients on healthy lifestyle requirements and wellness plans.
• Identify health risk factors and manage chronic illness with clients.
• Motivate individuals to achieve health goals and monitor progress.
• Support Medicare STARS, HEDIS, and HCC initiatives with providers.
