Job summary

Devoted Health is seeking an Advanced Practice Provider to join the Gold Care program. You will reinforce and adjust care plans, manage chronic conditions, and coordinate with an interdisciplinary team across geographies using telehealth.

Responsibilities include comprehensive assessments, medication reviews, and supporting quality metrics while ensuring patient priorities guide care decisions. A strong focus on whole-person care is emphasized.

Qualifications

• Active RN and APRN or PA license; multiple state licenses may be required.
• 3+ years as APP in primary care, house calls, internal medicine, geriatrics, or palliative care.
• Proficiency with telehealth tech and electronic health records (EHR).
• Active BLS at time of hire and maintained during employment.

Responsibilities

• Deliver exceptional care to a cohort of complex patients via telehealth and interdisciplinary team.
• Oversee medical management of patients using the 5M framework.
• Collaborate with PCPs, specialists, pharmacy, and family/caregivers to coordinate care.
• Conduct comprehensive assessments, post-acute transition support, and review medications and care quality metrics.
