POSITION SUMMARY
The case manager ensures the overall coordination and management of cases through assessment, service planning, linkage with services, and establishing collaborative relationships. They provide most services in the community and in families' homes. The case manager acts as the primary liaison between participants (children, parents, foster parents) and the various systems involved during their interaction with DCFS. Responsibilities include providing basic counseling/stabilization as needed, under supervision and consultation, communicating regularly with the supervisor and external service providers, and documenting all relevant information promptly.
