Pair Team is seeking a Bilingual Field Care Coordinator to deliver hands-on, in-home and community-based care coordination for members with complex medical, behavioral, and social needs in Irvine, California. You will travel regularly, onboard new patients, and provide care packages and health readings while navigating barriers to care.

Pair Team is expanding its partnership with one of our largest health plan partners through a new field-based care management pilot focused on improving patient engagement and access to care.
The Field Care Coordinator plays a critical role in bringing care directly to our members where they are, both at home and in the community. This role focuses on hands‑on, relationship‑based support through in-person visits, new patient onboardings, and field‑based outreach. Field Care Coordinators help members overcome barriers to care by assisting with everything from care plan follow‑up and blood pressure checks to grocery or care package drop‑offs.
The expectation for this role is to be in the field meeting patients three to five times per week. This is a full-time, field-based position requiring regular travel within Orange County.
Schedule: M-F; 9am - 5:30pm
Travel: 4 days a week with 1 work from home day *gas and mileage reimbursed
