Akido Labs, Inc. in California is seeking a Community Health Worker to support members with complex medical and social needs, helping them manage health and wellbeing through education, peer support, and shared experiences.

You will manage a caseload, create care plans with patients and clinical teams, build rapport to encourage positive behavior changes, accompany members to appointments, and connect them to local services while documenting encounters in our systems.

Qualifications

• High School Diploma or equivalent.
• Bilingual in English and Spanish strongly preferred.
• 2+ years of experience with CalAIM populations (homeless, SMI/SUD, formerly incarcerated, complex medical needs).
• Ability to work outdoors and independently as well as in a team.
• Strong communication and interpersonal skills.
• Proficient with documenting in case management software.

Responsabilités

• Manage a caseload of patients, complete assessments, develop care plans with patients and clinical teams, and carry out activities per the plan.
• Build rapport with patients to encourage positive behavior changes.
• Coach patients to minimize risks related to health conditions and behaviors.
• Accompany members to medical and social services appointments.
• Connect members to local, county and state services and advocate for them.
