Job summary

Hospital Support Services (HSS) in Baltimore, MD seeks a motivated bilingual Field Representative Application Counselor to guide uninsured patients through the Medical Assistance (MA) process in English and Spanish. You’ll meet patients in hospitals or after discharge at agreed locations and collect required documentation.

This field-based role covers Maryland hospitals with a main office in Baltimore's Little Italy.

Qualifications

• Bilingual proficiency in English and Spanish required; must be able to communicate with patients in both languages.
• High school diploma or equivalent required; some college or social services experience preferred
• Prior experience in case management, healthcare, or social services is a plus
• Strong organizational and time management skills
• Excellent communication and interpersonal abilities
• Comfortable working independently and managing multiple cases
• Basic computer skills; willing to learn case management software
• Valid driver’s license and reliable transportation required

Responsibilities

• Assist English- and Spanish-speaking patients with MA applications
• Collect required documentation for MA applications (e.g., Social Security card, photo ID, income verification, bank statements)
• Meet patients in hospitals or, after discharge, at a convenient location to complete applications
• Assist in locating and supporting homeless patients with applications
• Travel to the Baltimore office once per week at a minimum to: Submit paperwork; Enter cases into the database for payroll and tracking; Meet with staff for updates
• Maintain regular communication with the HSS Case Manager about case progress
