Job summary

Hospital Support Services (HSS) in Baltimore, MD, is seeking a Case Manager – Medicaid Eligibility to oversee eligibility determinations for hospital-referred patients. You will manage a caseload from intake to approval, perform screenings, collect documentation, and coordinate with hospitals and social services.

This full-time hybrid role offers a Monday-Thursday 9:00 AM – 6:00 PM schedule with remote Mondays and Fridays; on-site Tuesdays through Thursdays in Baltimore.

Qualifications

• Bilingual English/Spanish required.
• HS diploma or equivalent; healthcare admin experience preferred.
• Strong data entry and organizational skills.
• Ability to handle confidential information and multiple cases.

Responsibilities

• Manage a caseload of Medicaid eligibility cases from intake through approval.
• Screen eligibility and determine appropriate Medicaid category.
• Maintain accurate case records in the HSS database.
• Collect eligibility documentation from patients.
• Coordinate with hospitals, social services, and field staff.
• Submit medical records and track timelines for enrollment.
• Communicate with DSS caseworkers for approvals.
• Review cases for accuracy, compliance, and payment processing.
• Ensure documentation is complete and current.
