Job summary

PHC in Des Moines, IA seeks a Behavioral Health Therapist to join a multidisciplinary primary care team, providing assessments and brief and long-term therapy for children, adolescents, adults and families. This position offers in-person and telehealth options across Des Moines clinics, with flexible hours including full-time, part-time or PRN.

Spanish and English bilingual skills are preferred; licensure (LISW/LMSW, LMHC or LMFT) and a Master’s degree are required, with 90 days for required

Qualifications

• Licensed social worker or LMHC/LMFT in Iowa.
• Master’s degree in Social Work, Marital and Family Therapy, Clinical Mental Health Counseling, or related field.

Responsibilities

• Provides behavioral health care in accordance with PHC standards and collaborates with healthcare providers to deliver comprehensive services.
• Provides brief and long-term therapy to individuals and families in PHC clinics or via telehealth.
• Accepts internal referrals and provides timely feedback to referring providers.
• Makes referrals to community mental health resources as appropriate.
• Maintains accurate documentation and manages required data.
