Qualifications

• Associate's degree in health education, social work, education, public health, or related field.
• Two years' experience in health care, education, community outreach, preferably related to Maternal Child Health.
• Must obtain Infant Family Studies Certificate or CHW within 2 years of hire.
• First Aid and CPR certification preferred.
• Ability to pass criminal background check, pre-employment drug screen, and ECECD background check.
• Must have a valid non-restricted driver's license.

Responsibilities

• Maintain a caseload of the LCPAT Program in Luna County including new and existing cases; assess risk levels, develop client service plans and maintain compliance with agency policies.
• Conduct bi-monthly visits with program participants and childcare providers, provide resource network and screenings; deliver services in English and Spanish.
• Ensure that Parents as Teachers, ECECD and Luna County Policies and Procedures are followed.
• Promote the program and recruit participants.
• Ensure activities are implemented on-site and in-home to establish positive links between home and program.
• Track documentation and enter data on a software program; maintain confidentiality policies and HIPPA compliance.
