Qualifications

• Level 1: Associate's degree in Social Work or Behavioral Health with 18 credit hours in Social Work/Behavioral Health.
• Level 2: Bachelor's degree in Social Work or Behavioral Health with 18 credit hours in Social Work/Behavioral Health.
• One year experience in social service delivery or case management.

Responsibilities

• Provide resources and referrals during home visits to assist with family goal setting and self-sufficiency.
• Support families in becoming active participants in their children's education and community.
• Maintain ongoing documentation of communication with families and coordinate with other agencies as needed.
