Job summary

The Oregon Department of Human Services is seeking a Family Coach (Human Services Case Manager) in Grants Pass to empower families toward independence. You will conduct strength-based coaching, assess needs, coordinate services, and promote equity in access to resources.

The role requires English/Spanish proficiency, a degree in behavioral or social sciences or related field, and experience in human services or case management. A valid driver's license is required.

Qualifications

• Bachelor's degree in Behavioral Science or Social Science or closely related field or equivalent.
• Bachelor's degree in any field with 1 year human services experience.
• Associate degree with 2 years human services experience.
• Three years of human services related experience.

Responsibilities

• Empower families to achieve lasting independence through person-centered, strength-based coaching.
• Partner with ODHS programs and community agencies to ensure equitable access to resources.
• Engage families with assessments and collaborative goal setting to identify barriers and supports.
• Create actionable plans with adults to reduce reliance on public assistance.
• Develop individualized coaching and employment plans emphasizing skills and work experiences.
• Coordinate referrals to job training and community partners to improve employability.
• Use motivational interviewing to inspire confidence and persistence toward employment goals.
• Maintain up-to-date knowledge of employment services and resources to tailor referrals.
