Job summary

Qualifications

• Bachelor’s Degree in Social Work or related field is required.
• Minimum of 2 years of case management experience with at-risk children and families.
• Must be able to read, write, and speak English and Spanish.

Responsibilities

• Create, develop, and implement Family Hope Center programs to meet clients’ needs as outlined in each plan of service.
• Coordinate and provide case management services including enrollment, assessments, and referrals.
• Assist with arranging out-of-home placement or respite care as needed.
• Coordinate therapy services and crisis counseling as appropriate.
• Help participants access community resources and enroll in services.
• Develop relationships with schools, agencies, and community partners to expand services.
