Job summary

Pinnacle Family Services in North Carolina is seeking a Family Centered Treatment (FCT) Specialist to deliver evidence-based services in homes and communities. You will guide families through the structured four-phase FCT model and coordinate with medical, behavioral health, school, and community partners to ensure holistic care.

This role requires FCT certification within one year of hire, offers flexible scheduling, 24/7 on-call crisis support, and a strong benefits package as part of Health

Qualifications

• Bachelor's Degree required — in Human Services with a minimum of two years of experience working with youth or families
• Current FCT Certification is a plus, though not required — we'll help you get there
• Must meet qualifications as a Qualified Professional (QP) with experience serving children and adolescents with a primary mental health diagnosis
• Must have a valid driver’s license and access to a fully insured, private form of transportation

Responsibilities

• Deliver Family Centered Treatment services primarily in home and community settings, meeting families where they are — literally and figuratively
• Guide children and families through the four phases of the FCT model, helping them set goals, build skills, and move toward sustainable well-being
• Serve as a central point of coordination for each family's Treatment Team, collaborating with medical, behavioral health, school, and community partners to ensure seamless, whole-person care
• Develop, implement, and continuously refine individualized Service Plans in partnership with the families you serve
• Connect families to community resources, referrals, and aftercare supports that extend the impact of treatment beyond your time together
• Maintain timely, accurate documentation in the Electronic Health Record, keeping records that reflect the quality of care you provide
• Participate in Quality Assurance activities that help make our programs stronger for every family we serve
