Job summary

Sheppard Pratt in Gaithersburg, MD, seeks a part-time, day-shift, in-person Case Manager for the SCYS school-based mental health and social services program in collaboration with MCPS and DHHS.

Provide culturally and linguistically appropriate case management to students and families, linking them to housing, healthcare, employment, education, and other resources; document plans; coordinate with therapists, school staff, and partners; maintain confidentiality.

Qualifications

• Bachelor’s degree in Social Work (B.S.W.), Psychology, or related field.
• Experience working within collaborative school-based models preferred.
• Experience with immigrant populations & acculturation issues preferred.
• Valid state driver's license with 3 or fewer points at time of hire.
• English and Spanish fluency may be required depending on assignment.

Responsibilities

• Provide case management services to students/parents/families with economic and/or social needs.
• Use the County’s electronic records system to document service plans and update plans every six months.
• Maintain regular contact with clients (twice a month) and document within one business day of service.
• Maintain a caseload in line with contract terms and program expectations.
• Organize meetings, workshops, and events to develop partnerships between families and schools.
• Develop referral networks to promote a continuum of care.
• May conduct home visits and provide referrals as needed.
• Collaborate with therapist and school staff while maintaining client confidentiality.
