Job summary

Wellness Pointe in Texas is seeking a Thriving Texas Families Bi-Lingual Community Care Coordinator to deliver core services to eligible individuals, including pregnant mothers and guardians of children aged 0–3. The role includes crisis support, referrals, case management, and mentorship to improve health outcomes and family dynamics.

Qualifications

• Bachelor's degree in Behavioral Health or 2 years case management in Behavioral Health or CHW certification.
• Knowledge of prenatal care systems and postpartum services.
• Ability to conduct screenings, assessments, service plans, and follow ups.
• Valid Texas Driver's License, driving record and auto liability insurance.
• Reliable transportation.
• Proficient with MS Office (Outlook, Word, Excel).
• Motivated and able to work with diverse groups.
• Independent worker with initiative and team values.
• Mental and physical health sufficient for the job.
• Fluent in Spanish (reading and speaking).

Responsibilities

• Represent the program at community meetings and identify resources to meet client needs.
• Maintain program supplies, process approvals, and coordinate pickup.
• Assist leadership in monthly reports with all staff activities and demographics.
• Facilitate outreach, recruitment, intake, referrals, client cases, and group sessions.
• Provide basic needs support and conduct follow ups per policies.
• Ensure outreach materials are approved, organized, and culturally appropriate.
• Report to Lead Community Care Coordinator and Program Manager on goals and objectives.
• Engage patients to stay connected to the medical system during pregnancy and facilitate referrals.
• Communicate concerns to staff and assist in developing new program components.
• Maintain flexible schedule and may work irregular hours; perform other duties as assigned.
