Qualifications

• Bachelor's degree from an accredited college or university.
• One year of experience working with persons with developmental disabilities.
• Valid Oregon Driver's License or ability to reach service areas not accessible by public transportation.
• Equivalent combination of experience and training or demonstrated abilities.

Responsibilities

• Complete the Oregon Needs Assessment (ONA) through document review and interviews with individuals and families.
• Provide Services Coordination with person-centered planning and referrals for non-funded resources.
• Conduct intake and eligibility determinations by reviewing medical, school and psychological records.
