Pillars Community Health is seeking a detail-oriented administrative support professional to assist the Mobile Crisis Response team. The role involves triaging crisis case assignments, aiding hospital placements, and coordinating discharge appointments with direct client contact.

The position requires 1-2 years of direct case management in social services and bilingual English/Spanish ability. Travel to client locations may be needed, with mileage reimbursement.

Qualifications

• Bachelor's degree in counseling, psychology, social work, marriage and family therapy or related field required.
• 1-2 years of direct client case management experience in social service, behavioral health, community health outreach, or related field required.
• Bilingual (English/Spanish) required.

Responsibilities

• Provides administrative support to triage crisis response case assignments and facilitate hospital placements and discharge appointments.
• Assists with limited case management activities and maintains client contact across office, home, or community locations.
• Maintains electronic health records and logs to monitor program outcomes and KPIs.
• Outreaches to clients and families and discusses resources and continued treatment options.
