Job summary

Primary Care Coalition is seeking a Bilingual Community Resource Coordinator to connect underserved patients with health and social services and assist with referrals and visits. You will help patients access benefits and support behavioral health treatment within PCC’s Collaborative Care model.

You will conduct outreach, coordinate referrals, and document encounters in the EHR, working closely with care managers and partner agencies.

Qualifications

  • Bilingual English/Spanish required.
  • Excellent communication and interpersonal skills.
  • Experience with community resources navigation preferred.
  • Familiarity with HIPAA and patient confidentiality.
  • Ability to travel within the service area for home visits.

Responsibilities

  • Provide Patient Navigation and Support Services as part of the treatment plan.
  • Support behavioral health care managers with patient navigation and referrals.
  • Refer patients to community or social service agencies.
  • Complete home and community-based visits to help patients access benefits and services.

Job description

Hourly Rate Range: $24.00 to $26.00 per hour

Telework: This position requires four days of in-person work each week, including two to three days at health center sites and one day at PCC. Other in-person duties may include in person meetings at PCC or community-based visits with patients or partner agencies. The employee must attend designated face-to-face meetings and complete other assigned in-office duties. Note: PCC employees must reside in the following five states and District of Columbia: MD, VA, WVA, DE, PA. & DC. Relocation costs are not provided.

Primary Care Coalition (PCC) has an open position in the Montgomery Cares Behavioral Health Program (MCBHP) for a Bilingual Community Resource Coordinator (BCRC). Working as part of PCC’s Collaborative Care model, the BCRC connects underserved patients with health and social services that address basic needs and support behavioral health treatment. The BCRC helps patients navigate services, coordinates referrals, conducts outreach and resource research, assists other agencies in connecting community members to health care programs, and supports care managers with behavioral health-related activities and questions about community resources.

Essential Duties
  • Provide Patient Navigation and Support Services
  • Support behavioral health care managers by providing patient navigation and support services to patients as part of their treatment plans.
  • Refer patients to community or social service agencies.
  • Complete home and community-based visits to help patients access benefits and services. Examples include accompanying patients to county government offices to apply for benefits or delivering food from referral partners to homebound patients who cannot access transportation because of advanced age, illness, or disability.
  • Assist with applications (e.g., SNAP, Montgomery Cares).
  • Document all patient encounters accurately, appropriately, and promptly in the electronic health record and case management system while maintaining compliance with HIPAA.
  • Collaborate with partner organizations and other departments at PCC
  • Maintain positive relationships with healthcare and social service providers, including colleagues at PCC in other departments dedicated to helping patients and their families access healthcare and other services.
  • Participate in community events and networking, including making presentations about PCC’s healthcare access programs.
  • Share information with MCBHP and PCC team members on community resources that are available to patients.
  • Serve as the point of contact and manage referrals for the MCBHP for programs that accept referrals from MCBHP, such as for the annual Holiday Giving Program.
  • Support Care Managers in delivering behavioral health interventions
  • Assist Care Managers with behavioral health screenings for anxiety and depression.
  • Support patients with health literacy challenges (e.g. teaching patients how to make follow-up appointments at primary care clinics).
  • Reinforce recommended behavioral health changes by sharing basic nutrition and healthy-lifestyle information and supporting patients as they work toward care-plan goals.
Additional Duties
  • Attend training to improve skills in appropriate areas and learn about new resources available for patients.
  • Deliver prepared information about programs and participate in networking activities with external and internal stakeholders.
  • Contribute to program publications and PCC committees and cross-departmental workgroups.
  • Participate in meetings of MCBHP, PCC, and health center staff as appropriate.
  • Other duties as assigned.
Skills And Abilities
  • Professional proficiency in spoken and written English and Spanish, including the ability to communicate clearly with patients and document services accurately is required.
  • Team player with strong interpersonal skills, including comfort with initiating and building rapport with patients and community partners.
  • Effective time management and problem-solving abilities.
  • Knowledge of community resources and benefits and ability to coordinate referrals.
  • Ability to deliver prepared information about programs and participate in networking activities with external and internal stakeholders.
  • Computer literacy and proficient or able to learn and utilize electronic health records and Microsoft Office Suite.
Education And Experience
  • High School graduate or GED required.
  • At least one year of experience in health, human services, customer service, patient support, or a related setting preferred. Relevant volunteer, internship, or community-based experience may be considered.
  • Experience working or volunteering with diverse, low-income, or immigrant communities preferred.
  • Knowledge of behavioral health or medical issues preferred.
  • Community Health Worker licensure preferred.
ADA Requirements
  • This role routinely uses standard office equipment such as computers, telephones, photocopiers, filing cabinets and fax machines.
  • This is largely a sedentary role; however, some filing may be required.
  • This would require the ability to lift files, open filing cabinets and bend or stand as necessary.
  • Requires ability to learn new software applications as necessary.
  • Must be able to lift up to 20 pounds.
