Job summary

Primary Care Coalition (PCC) in Montgomery County is seeking a Bilingual Community Resource Coordinator to connect patients with health and social services. You will navigate benefits, coordinate referrals, and support care managers within the Collaborative Care model to improve access to behavioral health treatment.

The role requires English and Spanish proficiency, strong interpersonal skills, and the ability to work in a hybrid environment with in‑person visits and health center partnerships.

Qualifications

• High School graduate or GED required.
• At least 1 year experience in health, human services, customer service or related setting preferred.
• Experience with diverse, low-income, or immigrant communities preferred.
• Knowledge of behavioral health or medical issues preferred.
• Community Health Worker licensure preferred.

Responsibilities

• Provide patient navigation and support services to patients as part of treatment plans.
• Refer patients to community or social service agencies and complete home visits.
• Collaborate with PCC departments and partner organizations; manage referrals for programs that accept referrals from MCBHP.
