POD Health is seeking a Field-Based Community Outreach Representative to drive growth by developing referral networks with medical offices, senior centers, and community partners across Colorado. You will educate partners about POD Health services, identify eligible patients, and manage SOC referrals in collaboration with internal teams.

• Bilingual fluency in Chinese (Mandarin and Cantonese) required.
• Valid driver’s license and reliable transportation for local travel.
• Community-based business development, Medicare or value-based care preferred.
• Strong understanding of Medicare populations and community healthcare dynamics.
• Proven ability to build provider relationships and convert referrals into enrolled patients.
• Excellent interpersonal, communication, and presentation skills.
• Cultural competency and experience working with diverse communities.
• Proficiency in Microsoft Office and Salesforce CRM (or comparable systems).

Responsabilidades

• Build and maintain relationships with primary care practices, specialist offices, community health organizations, senior centers, and advocacy groups.
• Educate referral partners on POD Health services, eligibility requirements, and patient benefits.
• Drive SOC (Start of Care) volume by generating and following up on qualified referrals.
• Plan, coordinate, and host community and provider-focused events (health education workshops, enrollment events, senior center presentations, provider lunch-and-learns).
• Represent POD Health at community events, health fairs, provider meetings, and networking opportunities.
• Collaborate with intake, clinical, and operations teams to ensure timely follow-up and SOC conversions.
• Track and document outreach activity, referrals, and follow-ups in Salesforce within required timelines.
• Support growth initiatives, pilot programs, and special outreach campaigns.
• Maintain compliance with Medicare marketing guidelines and organizational standards.
