Job summary

Whole Systems Learning, a nonprofit in Long Beach, is seeking a Community Health Worker (Spanish-Speaking Preferred) to support individuals returning home after incarceration through the Reentry Intensive Case Management Services (RICMS) program.

The role builds relationships, conducts outreach, and connects participants to housing, employment, healthcare, and other supports; bilingual Spanish-speaking candidates are strongly preferred, and a compassionate, resourceful advocate is essential.

Qualifications

• High School Diploma or GED required.
• 1+ year in social services, case management, or related field.
• Strong communication, interpersonal, and organizational skills.
• Valid California Driver's License and travel within service area.

Responsibilities

• Build trusting relationships with participants.
• Conduct outreach, assessments, and service planning.
• Identify goals, needs, and barriers to success with participants.
• Develop and monitor individualized service plans.
• Connect to housing, employment, healthcare, and education resources.
• Coordinate referrals and track progress per contract requirements.
