Urban Health Plan, Inc. is looking for a Community Health Worker to join our Perinatal Infant Community Health Collaborative (PICHC). This hybrid position offers a blend of onsite and remote responsibilities, primary focusing on maternal and infant health challenges.

The Community Health Worker will facilitate outreach, education, and supportive services for families in the Bronx, NY. A passion for health and an Associates Degree are required, along with computer literacy and bilingual
