Job summary

Zócalo Health Medical Group is seeking a Community Health Worker to empower Latino communities by navigating health and social services. You will educate patients, coordinate appointments, and connect them with essential community resources to improve access and outcomes.

The role involves building trust, organizing events, and supporting care plans across various settings, with travel as needed and potential remote components.

Qualifications

  • Fluent in English and Spanish (verbal and written).
  • High school diploma or GED required.
  • CA CHW certification or 2,000+ hours CHW work with certification planned within 18 months.
  • 1–3 years healthcare navigation experience or 2–5 years community work.
  • Experience with Google Workspace and EHR documentation preferred.
  • Motivational interviewing training is a plus.
  • Willingness to travel and be vaccinated as required.

Responsibilities

  • Register and enroll patients and organize community engagement.
  • Conduct outreach (virtual and in-person) for appointments and intake.
  • Provide care navigation, scheduling, referrals, and follow-up coordination.
  • Assess social determinants of health and enroll in SDOH programs.
  • Collaborate with care teams to support plans and triage needs.
  • Document interactions across systems and tools.
  • Participate in community events and educate patients on health topics.
