Qualifications

• Minimum of one year of experience working with children and/or youth with disabilities (per waiver Manual)
• Licensed or certified under Wis. Stat. § 457.08 or approved alternative qualification by DHS
• Must have a valid Wisconsin driver’s license and acceptable driving record
• Ability to interpret and translate when needed, or via SFCC approved provider
• Ability to establish and maintain client confidentiality and professional relationships
• Strong knowledge of waiver rules, deadlines, and ISP processes

Responsibilities

• Coordinates delivery of CLTS services across agency lines
• Serves as single point of contact to help families obtain required services
• Oversees caseload of 50-55 clients with average 8 units per month per child
• Develops, reviews and updates Individual Service Plans (ISPs)
• Completes PAS, PFA and documentation for plan updates
• Maintains timely case notes (within 48 hours) and compliant paperwork
• Maintains regular contact with families and ensures services are delivered as described
• Assists with interpretation/translation as needed
• Performs other duties as assigned
