By the Bay Health in San Francisco seeks a Clinical Team Manager Bi-lingual (Cantonese) to lead the clinical team, coordinate high quality patient care through an interdisciplinary approach and uphold our standards of care and ethics.

The role oversees patient satisfaction, quality scores, and staff development in a hybrid work setting, with four onsite days and one remote day weekly, and requires leadership and language/cultural skills to serve diverse patients.

Competenze

• BSN in Nursing or Master’s degree in social work from a school accredited by the Council on Social Work Education.
• Minimum of two years of nursing or social work practice in home health, hospice or community health within the past five years.
• Five years supervisory or progressive management experience preferred.

Mansioni

• Provide guidance to the team members in regular transdisciplinary team meetings; develop, review and revise the patient/family plan of care; facilitate problem-solving sessions; and deliver team support.
• Assess daily staffing needs and redistribute resources based on census to maximize patient care.
• Periodically audit selected clinical records for documentation related to supervision and regulatory standards.
• Coordinate transdisciplinary team meetings and supervise planning, coordination, and delivery of interventions.
• Conduct supervisory visits for clinical observation in accordance with policy, accreditation and standards.
