By The Bay Health, a California non-profit, seeks a Clinical Team Manager Bi-lingual (Cantonese) to lead our hospice clinical team in San Francisco. The role emphasizes patient-directed care, interdisciplinary collaboration and maintaining high-quality care and regulatory compliance.

The position is full-time with a hybrid schedule: four days onsite and one day remote, supporting a culturally diverse patient population with multilingual staff. Competitive pay and benefits are offered.

Qualifications

• BSN in Nursing or MSW from a CSWE-accredited school.
• Two years nursing or social work experience in home health, hospice or community health.
• Five years supervisory or progressive management experience preferred.

Responsabilités

• Provide guidance to the team members in regular transdisciplinary team meetings; the development, review and revision of the patient/family plan of care; facilitation of problem solving sessions; and delivery of team support.
• Assess daily staffing needs and implements re-distribution of resources based on census to maximize patient care.
• Periodically audit selected clinical records for documentation concerning HHA supervision, visits/matching orders, and other areas related to regulatory compliance and enter findings in audit database. Hold staff accountable to standards.
• Coordinate transdisciplinary team meetings and supervise the planning, coordination, and delivery of the transdisciplinary team interventions.
• Conduct appropriate supervisory visits to allow for clinical observation of performance of assigned team members in accordance with agency policy, accreditation, and regulatory standards.
