Job summary

Heartshare is seeking a Case Planner to assist each child in achieving permanency and safety while in care. You will coordinate services, liaise with ACS and community resources, and work with medical, mental health, school personnel and clinicians to support reunification.

The role includes contact with families, community partners and court hearings, with a focus on child welfare outcomes and prevention of trauma. Bilingual preferred.

Qualifications

• Bachelor's degree required.
• Excellent written, oral, organizational and interpersonal communication skills.
• Proficiency in Microsoft Office and related tools.

Responsibilities

• Creates service plans with child, family and foster family addressing safety and permanency needs.
• Monitors safety and progress via case notes and documentation.
• Schedules and supervises mandated visits; conducts interviews with child and families; visits relevant sites.
• Attends supervision meetings and Family Court hearings; provides status updates.
• Collaborates with Integrated Services to monitor medical and mental health needs.
• Uses data to measure performance and outcomes.
