Job summary

Cardinal McCloskey Community Services is seeking a Case Planner for the Prevention and Special Medical Program to support families at risk of foster care placement due to substance use or mental health disorders. You will assess needs, develop service plans, and guide families through a three-phase treatment model.

This role requires a BSW or bachelor’s degree in human services, knowledge of the child welfare system, and strong collaboration with internal and external providers.

Qualifications

• BSW or bachelor’s degree in human services.
• Some knowledge of the child welfare system.
• Fluency in English and Spanish is a plus.

Responsibilities

• Guide families through a three-phase treatment model: Initial, Baseline, and Stabilization.
• Conduct comprehensive assessments and develop individualized service plans.
• Coordinate multidisciplinary interventions tailored to each family's needs.
• Build meaningful relationships with families over an average case duration of 12 months.
• Collaborate with internal and external providers to ensure safety, stability, and well-being.
