Job summary

Kids for the Future in San Diego, CA is seeking a Case Manager to support children, youth and families through coordinated case management and rehabilitative services.

Work week is 40 hours, Monday–Friday, 8:00 am to 4:00 pm. Must be bilingual in Spanish and have a high school diploma with relevant experience in public mental health.

Qualifications

• Bilingual Spanish is required for this position.
• Experience working with children/adolescents with emotional disturbance in public mental health.
• Ability to assess, design treatment plans and document accordingly.
• Organize and prioritize multiple concurrent assignments and meet productivity goals.

Responsibilities

• Promote healthy development and positive outcomes for families through case management and rehabilitative strategies with a team.
• Assist clients and families to build upon natural support systems and link to service agencies.
• Provide supportive services to maintain continuity of treatment and connect to resources.
• Connect clients/families to medical homes, clinics, schools, and community services.
• Advocate for the client and family.
• Create and maintain documents in electronic and paper records.
