Job summary

Responsibilities

Assess, plan, implement, and coordinate case management activities telephonically and in person.
Evaluate members' medical needs to facilitate overall wellness.
Manage a caseload including supportive and medically complex members.
Develop proactive action plans to address issues and improve outcomes.
Coordinate services (prior authorizations, PCP and provider contacts, medication review, community resources).
Travel 50–75% for face-to-face member visits within Miami-Dade County.
