Job summary

CVS Health in Florida is seeking a Bilingual Case Manager Analyst - Field to support member health outcomes by coordinating care, providing education, and facilitating access to services. The role requires residency in North Miami, Miramar, Aventura, Pembroke Pines, Hialeah, or Flagler and travel up to 75%.

You will collaborate with internal teams and providers, monitor care plans, ensure regulatory compliance, and promote engagement and informed decisions.

Qualifications

• Bilingual English & Spanish required; must pass language assessments.
• Travel up to 75% with mileage reimbursed per policy.
• Must reside in North Miami, Miramar, Aventura, Pembroke Pines, Hialeah, Flagler.
• Minimum 1 year in behavioral health or long-term care setting.
• Proficiency in Microsoft Office Suite and computer use.

Responsibilities

• Coordinate care and provide education to support member health outcomes.
• Facilitate access to healthcare services and address barriers to care.
• Monitor care plan progress and ensure regulatory compliance.
• Collaborate with internal teams, providers, and external resources to improve quality of care.

Bachelor’s Degree or non-licensed Master’s level clinician
