Qualifications

• Bilingual English/Spanish required with passing language assessments.
• Must reside in Miami-Dade County, FL and be willing/able to travel up to 75% of the time.
• At least 1 year in a behavioral health or long-term care setting.
• Proficiency in Microsoft Office Suite.

Responsibilities

• Coordinate care, provide education, and facilitate access to appropriate healthcare services.
• Assess member needs, identify risks, and develop and implement effective care plans using care management tools and data analysis.
• Collaborate with internal teams, providers, and external resources to address barriers, improve quality of care, and ensure services align with member benefits.
• Promote member engagement, informed decision‑making, and healthy behavior changes through communication and coaching.
• Monitor care plan progress, document activities, and ensure compliance with regulatory standards, company policies, and quality management guidelines.
