Job summary

Nourishing Hope, a Chicago nonprofit, seeks a bilingual Case Manager to connect neighbors with critical supports, maintain records, and ensure respectful service aligned with our core values. This onsite role supports social services programs and requires strong communication and teamwork.

The position is primarily in-person with some remote work and duties at multiple sites, including Sheridan Market. A commitment to food security and equity is essential for success in this role.

Qualifications

• Bachelor’s degree in social work or related field.
• 1+ year of work experience in social services environments.
• Bilingual Spanish required.
• Proficient with Microsoft Office and Google Suite.
• Experience with Salesforce CRM and data quality.

Responsibilities

• Assist neighbors with connecting to supports (transportation, housing, benefits, healthcare).
• Maintain accurate case records in Azzly and Salesforce and track outcomes.
• Engage neighbors, volunteers, staff, and stakeholders with professionalism.
• Coordinate outreach via phone and email and manage scheduled appointments.
• Collaborate with Mental Wellness & Social Services team to align priorities.
