Job summary

The Center in San Diego seeks a Case Manager to provide case planning, outreach to eligible participants, assessments, and crisis intervention for LGBTQ affirming wraparound services. You will work with a trauma‑informed, goal‑oriented team to help resolve housing stability for program participants.

On-site in Hillcrest, SD, the role requires 1 year of homelessness-related case management, fluency in English and Spanish, and strong MS Office skills.

Qualifications

• 1 year of experience providing case management services to people experiencing homelessness or at risk of homelessness.
• Fluency in English and Spanish required.
• Comfortable working in PC/Windows-based environments with strong MS Office proficiency.
• Demonstrated ability to manage multiple tasks with shifting priorities.
• Competence in strength-based, trauma-informed, and Motivational Interviewing techniques.

Responsibilities

• Conduct intakes, assessments, and evaluations with new clients.
• Provide links to mental health, substance abuse, educational, career development, and other resources.
• Provide ongoing case management, crisis intervention, and stabilization services to participants.
