Job summary

La Familia is seeking a Bilingual Case Manager in Hayward, CA. You will assess client needs, develop collaborative plans, and coordinate housing, entitlements, and health care access while maintaining documentation.

Work under SEIU 1021 with scheduled annual base-pay increases; requires a BA in a mental health field, 2 years’ experience, bilingual English/Spanish, and a CA driver’s license. Hybrid work options available.

Qualifications

• BA degree in mental health, community outreach or related field.
• 2+ years in mental health or community outreach.
• Bilingual Spanish/English - proficiency in speech and writing.
• Valid CA driver's license and ability to travel.

Responsibilities

• Provide assessments of clients’ needs (housing, daily activities, counseling, entitlements).
• Develop and implement collaborative treatment plans; coordinate with family and providers.
• Assist with housing placement, entitlements, and access to health/dental care.
• Maintain timely clinical documentation and report on key metrics.
