Job summary

Sasha Bruce Youthwork seeks a bilingual case manager for PASSAGES, a transitional housing program in Washington, DC. The role focuses on care coordination, social service navigation, and client support in a multi-disciplinary team.

You will conduct counseling, develop service plans, connect clients to benefits and resources, and ensure timely documentation while working evenings or weekends as needed to support youth toward stability.

Qualifications

  • Bachelor's Degree in Social Work, psychology, or a human services related field OR three years of experience working with at risk youth.
  • MUST be bilingual (Spanish/English); history working with Latinx populations a plus.
  • Familiarity with social services and support agencies in the District.
  • Strong communication and organizational skills required.
  • Reporting to the Program Manager, this is a full-time, exempt level position.
  • Candidates must possess a valid driver’s license, reliable transportation, pass required clearances.

Responsibilities

  • Complete funder admission processes with clients, including intake and SPDAT assessments.
  • Create, implement and monitor individual service plans to assist clients towards self-sufficiency.
  • Connect clients with social services, vital documentation, benefits and resources.
  • Conduct individual counseling sessions with clients on caseload.
  • Support activity schedules, life skills workshops and trainings for clients.
  • Provide crisis intervention and on-site counseling when appropriate.
  • Perform regular house checks, monitor facilities, and enforce safety policies.
  • Maintain confidential case files and accurate service documentation within 24–48 hours.
  • Participate in team meetings, trainings, and outreach; ensure trainings are completed annually.
  • Evenings and weekends availability as needed.
  • Other duties as assigned; commitment to ending youth homelessness and racial equity.

Today, its programs in DC and Prince George’s County, MD, reach over 6,500 runaway, homeless, abused, and neglected youth, and their families.

About PASSAGES

PASSAGES is an extended transitional housing program with shared apartment units in ward 8. The program is meant for chronically homeless youth up to the age of 24 who can stay up to six years depending on their age and are supported with intensive case management services in order establish a level of stability and self-sufficiency.

Position Summary

Under the Program Manager, the bilingual case manager will provide care coordination, social service navigation and support to clients in PASSAGES.

Qualifications

  • Candidates must possess a valid driver’s license and have reliable transportation; pass the required clearances; and provide any additional information regarding previous records, if necessary.
Duties and Responsibilities
  • Complete all necessary funder specific admission processes with clients when referred to PASSAGES including intake, program rules and policies (confidentiality, bullying, release of information, etc.); ensure SPDAT assessments are up to date.
  • Using a competency based framework and in collaboration with other staff at PASSAGES, create, implement and monitor an individual service plan to assist clients in meeting their goals towards self sufficiency.
  • Connect clients with appropriate social services and actively support them in the navigation of services including vital documentation, food supports, relevant benefits, etc.; connect clients with relevant resources to their individual service plans and actively support them in cultivating those relationships.
  • Conduct individual counseling sessions with clients on caseload.
  • Support the reporting, planning and implementation of activity schedule at the program including various life skills, workshops and trainings on relevant topics to clients.
  • Support or provide crisis intervention and on-site counseling when appropriate.
  • Perform regular house check inspections inside the building and in the area immediately outside; monitor facilities and enforce program rules for client safety.
  • Help provides office coverage, answer and record all phone calls and visitors; complete service documentation in client case records within 24-48 hours of the service; maintain accurate, complete, and confidential case files, written documentation, reports to track client progress and outcomes.
  • Participate in regular team consultation meetings, staff meetings, trainings, conferences, regular supervision and community outreach events as required; taking special care to ensure that required trainings are completed annually.
  • Availability in evenings and weekends as needed.
  • Other duties as assigned by the Program Manager, the Chief of Programs or their designee; and.
  • Must have a commitment to ending youth homelessness, racial equity, inclusion, and issues central to Sasha Bruce Youthwork’s mission.
