Job summary

Sasha Bruce Youthwork, Inc. in Washington, DC is seeking a part-time Case Manager for the Transitional Housing Program. You will support clients, assess needs, and coordinate services to help families exit emergency shelters and move toward stable housing.

This 20-hour-per-week role requires a Bachelor’s in Social Work or related field (or 3 years in human services), crisis intervention skills, and strong communication. The position offers a mission-driven environment and benefits.

Qualifications

  • Bachelor’s Degree in Social Work, Counseling, psychology, or related Human Services field preferred OR 3 years’ experience.
  • Competency in crisis intervention and de‑escalation; and,
  • Strong communication and organizational skills required.

Responsibilities

  • Completes all necessary funder specific admission processes with clients when referred to the Transitional Housing Program including program intake, program rules and policies (confidentiality, bullying, release of information, etc.), SPDAT, etc.
  • With the support of the program manager and led by the client, creates an Individual Case and Service Plans; all plans must include a goal regarding exit to permanent housing.
  • Connects clients with appropriate social services, benefits and local referrals to mental health services, behavioral health treatment, housing, employment, etc.
  • Conducts individual counseling sessions with clients on caseload; including regular reviews of Individual Case and Service Plans.
  • Supports the planning and implementation of activity schedule at the program including various life skills workshops and trainings on relevant topics to clients as well as bringing outside vendors to facilitate.
  • Support or provide crisis intervention when appropriate;
  • Perform regular unit inspections as well as in the area immediately outside; monitor facilities and enforce program rules for client safety including but not limited to fire drills;
  • Help provides office coverage, answer and record all phone calls and visitors;
  • Complete service documentation in client case records within 24-48 hours of the service; maintain accurate, complete, and confidential case files, written documentation, reports to track client progress and outcomes;
  • Participate in regular team consultation meetings with the Program Manager;
  • Complete 40 hours of training annually;
  • Participate in staff meetings, conferences, regular supervision and community outreach events as required;
  • Other duties as assigned; and,
  • Must have a commitment to ending youth homelessness, racial equity, inclusion, and issues central to Sasha Bruce Youthworks mission.

Program Summary

SBY’s Transitional Housing Program offers service-enriched apartment-style living for families exiting emergency shelters in the District of Columbia. Initiated in collaboration with the Community Partnership for the Prevention of Homelessness, SBY grants ten families exiting city shelters seamless access to transitional housing units at 134-138 Wayne Place, SE. Residents receive a wide range of housing, case management, employment assistance, life skills education and other positive youth development services. Continuously updated, individualized service plans for each resident are geared towards the overall goal of assisting families in transitioning to independent or subsidized permanent housing. This program is only meant to last 2 to 2 and a half years.

Job Summary

The Case Manager will report to the Program Manager of the Sasha Bruce Transitional Housing Program. They will provide support to clients and work with relevant staff to assess and identify needs and appropriate services to ensure success of the program.

This position is 20 hours weekly Monday – Friday 10 AM -6 PM.

Qualifications
  • License/Certification: Bachelor’s Degree in Social Work, Counseling, psychology, or another relevant Human Services field preferred OR 3 years’ experience in the human service field preferably working with adolescents and/or young adults required;
  • Competency in crisis intervention and de‑escalation; and,
  • Strong communication and organizational skills required.

This is a part-time position.

All candidates must possess a valid driver’s license and have reliable transportation; pass the required clearances; and provide any additional information regarding previous records, if necessary.
